Cook County Service Area
We provide the following services: Skilled Nursing, Physical Therapy (PT), Occupational Therapy (OT), Speech Therapy (ST), Home Health Aide (HHA), and Medical Social Worker support in the following communities:

- Lincolnwood
- Evanston
- Des Plaines
- Bridgeview
Kane County Service Area
We provide Skilled Nursing, Physical Therapy (PT), Occupational Therapy (OT), and Speech Therapy (ST) in:

Service Limitations
- We do not provide care for patients under 18 years of age.
- We do not offer psychiatric or behavioral health services.
- We do not accommodate patients who require twice-daily visits.
- We do not provide home infusion services.
